General Information for Entertainers Network Applicant

Application for Entertainers Network Participation

Yes, I am interested in participating in your Annual Membership in the Entertainers Network. Please enter my order for full membership at a cost of $99.00. I have submitted my order via your secure order form with my credit card or enclosed a money order or check for $99.00 (please note: checks must clear the bank before membership is initiated). I understand that I may terminate my membership in the Gateway Emporium Mall, at any time, if I am dissatisfied with services, for a full and complete refund.

We believe strongly in the services we provide and in the members we serve. Absolutely no hidden costs of any kind are involved. Your check, money order or credit card orders will not be accepted, unless you are accepted as a member.

Upon acceptance, you will receive your voice mail box assignment, which will be faxed to you usually within 24 hours. Once you receive your voice mail assignment, you are on-line with our system. You will receive a 30 minute teleconference training session to show you the many features of our system. Our system is more than just a voice mail system, but truly a networking system that interacts with our promotional efforts.

I,______________________________________________, agree to pay an annual membership of $99.00, which is completely refundable at any time for participation in The Entertainers Network. This fee only helps to cover the costs of the services offered to me by Entertainers Network, including toll-free voice mail services, advertising, promotional services, mailings and trade show expenses. I will be paid a commission rate of 25%, based upon the services used by my referral(s) (for the referral's 1st month's paid fees) and up to a 10% residual rate per month thereafter, to be paid on a quarterly basis, for services owned and provided by Dana Technologies, including web
site hosting and voice mail services, but not including paging and long distance services. Should I decide to offer these services, my commission rate will be determined by the agreement that Impact Networks has with the suppliers, such as BTI, UUNet and PageMart.

I understand that Entertainers Network and Impact Networks each have the authority to terminate my membership to the Entertainers Network at any time, due to unsatisfactory performance, unfair business practices, client complaints, abuse of services, unethical or illegal activity and failure to pay for services. By being an annual member, I understand that I have access to support services being offered by Entertainers Network. I also understand that I will be assigned a toll-free voice mail box number, where leads are placed, is so needed, member announcements and communications from clients, potential and otherwise. I will be allowed to use the services as part of my business and in
the process of conducting my business. I further agree not to use this mail box number or any other services owned or provided by Dana Technologies for any false, misleading or otherwise unethical or illegal business or personal activity.

Attach any comments to this agreement and sign the comment letter ( if applicable). A signed copy will be returned to you, upon acceptance in the Impact Networks.
